Board of Directors performance evaluation

I am looking for effective examples of performance evaluation forms and program guidelines for use with Boards of Directors. If any member has had positive experiences with such documents or know of public sources, I would appreciate learning of them. The Board I am working for is of a public, for-profit firm.
